Assist adolescent and young adult members to receive early, holistic, comprehensive, culturally and linguistically competent transition planning to prepare them for service and benefit changes that will occur following their 21st birthday and the transition from a pediatric to adult health care model.
Education and Experience
Requires a Masters of Social Work degree.
Two (2) years of experience working with vulnerable populations and people with chronic or complex conditions.
Knowledge of STAR+PLUS benefits/services, community resources, LTSS, waiver programs, educational IEPs, vocational programs desirable.
Bilingual English and Spanish preferred.
